What are Lab and Service Agreements?

A lab agreement is a legally binding agreement between the University and an external entity, such as a private corporation or federal agency.

A service agreement sets forth the terms and conditions pertaining to specific goods or services to be provided by the University.

When do I need to get an Agreement?

Fill out this CRITERIA SHEET provided by the Dean's office helps define whether it is an agreement or research.

How do I get started?

  1. Fill out the Criteria Sheet above
  2. Write 2 paragraphs outlining your proposed activity
  3. Contact Olga Hernandez: (858)246-0034, ohhernandez@ucsd.edu. She will help you to determine what forms you will need to complete before preparing the appropriate agreement.

If the project is with a for profit entity, you will need to complete a 700-U form which is required by law.
If there are positive disclosures on the 700-U form, the 700-U addendum must also be submitted.
The 700-U conflict of interest form(s) are the only forms that must be submitted in hard copy with the faculty member's original signature to comply with state law.
