Plan for your Arrival

Please plan to arrive on time. This will allow time to park and walk to the clinic. It will also provide us adequate time to process your insurance and billing information correctly. For clinic visits, please arrive a minimum of 15 minutes before your scheduled appointment to allow for registration.

What to Bring for Your Visit

To make your visit easier, remember to bring:

  • Health insurance card (PPO, HMO, Military TriCare with Military ID)
  • Immunization Record for your Child/Teenager
  • Picture identification, such as a driver's license (not required but a good idea to bring).
  • Please Download, Print, and Complete the following forms, and bring them with you to your first visit.
