Shift Models

Wards Model

Residents working at Rady Children’s Hospital of San Diego will join one of 4 teams. Each team will be made up of one PGY-3 or PGY-4 resident, one PGY-2 resident, and two PGY-1 interns. Every team will admit patients to the General Pediatric service. In addition, our Yellow Team will admit Nephrology and Endocrinology patients, our Red Team will admit Neurology patients, our Green Team will admit Gastroenterology patients, and our Blue Team will admit Pulmonology patients. This pairing ensures broad exposure for residents, while still matching specific hospitalist and subspecialty attendings to each team and thereby maintaining excellent teaching.

In addition to the teams mentioned above, each resident on the wards will belong to a separate nighttime team. These night teams will be comprised of one member of each of the daytime teams so that every “color” is represented at night. During the 4 weeks of a wards rotation, each resident will spend one week (Monday through Friday) and one weekend (Saturday and Sunday) working overnight from 7:30 pm until 9:30 am. All overnight shifts will be with the same nighttime team and the weekend nights will be separated from the weekday nights so that no resident will do more than 5 consecutive night shifts. Weekends will be similarly divided so that admitting teams will be present during the daytime hours and a separate overnight team will admit patients from 7:30 pm until the following morning. Rounds with attending staff will always take place between 7:30 am and 9:30 am, so that residents will be able to present new patients and receive teaching after their overnight shifts. Patient-loads will be capped, so that no resident will be expected to admit more than 8 patients overnight or to carry more than 10 patients during their daytime shifts.

NICU Model

Residents working in the Neonatal Intensive Care Unit at the UCSD Medical Center will also be on a shift model. The NICU team will typically consist of one PGY-3 level resident and three PGY-1 level residents for a total of four residents on at any given time. During the 4 weeks on the NICU rotation, each resident will spend one week (Sunday through Thursday) and one weekend (Friday and Saturday) working overnight from 6:30 pm until 8:30 am. Overnight admissions will be presented by the overnight admitting resident to the daytime team of residents, fellow and attending. This model allows for nighttime residents to get adequate teaching during their one week on nights also. Each resident will carry up to a maximum of 10 patients during their daytime shifts.
